To register, please bring your medical card to reception. This must be signed by you. If you have lost your card or have been living outside the UK for more than one year please advise a member of our reception staff who will issue you with an appropriate registration form.

If you are registering for the first time in Northern Ireland or are returning to Northern Ireland after a period away of more than one year, you will be asked to complete Form HS22X.

We ask all new patients to attend for a new patient medical - this enables us to obtain relevant information while waiting for your previous medical records to reach us. It also gives you the opportunity to meet one of the practice doctors or practice nurse and have a health check. You will be invited to make an appointment when we have processed your registration documents.  This normally takes a maximum of 6 weeks.

To prepare for your registration you can download the medical questionnaire, print it out and fill it in.  This will speed up the registration process when you first visit the surgery:

Temporary Residents

The Practice contracted boundary area is an 8-mile radius (approximately).

New patients are welcome from within the practice area, which excludes Aghalee, Edenderry, Carryduff, Ballynahinch, Dromara, Lower Ballinderry, Glenavy and part of Finaghy (see map below).

The boundary line between the Practice and Belfast is the “Finaghy Cross Roads”. This line also excludes Carryduff Village.

The boundary line between the Practice and Moira is the “roundabout at the top of the village”.
